Welcome. Philanthropi Charitable is the 501c3 nonprofit that administers the Ullico Employee Hardship Relief Fund.  We have partnered with the web platform Submittable for online form generation. In order to submit a grant, you will be prompted to create a free Submittable account which will allow you to save drafts and view your application status once completed. Please make sure you are using Chrome, Safari or Firefox browsers.  
The form will take approximately 30 minutes to complete and requires you to provide the following information:

  1. Your Ullico Employee ID-The 4 digit Employee ID number is listed on the right side of the employee’s profile in the Dayforce HRIS system.
  2. Date in which the qualified event occurred
  3. Documents and receipts for expenses incurred as a result of the qualifying event for which you are requesting funding

Make sure you answer all required fields and accept the Terms of Use prior to submitting your grant application.  At any time during the process, you can save your application in draft mode by scrolling to the bottom of the form and clicking "Save Draft."  Once you "Submit” your completed application, you will need to contact our support team directly to make any further changes.  
Please note, this application will be treated in a confidential manner by Philanthropi Charitable, and only data required to confirm an employee's eligibility along with non-identifying information, will be shared with your employer.
