Crisis Assistance Employee Hardship Relief Fund

Welcome. Philanthropi Charitable is the 501c3 nonprofit that administers the Crisis Assistance Ministry Employee Relief Fund and we have partnered with the web platform Submittable for online form generation. In order to submit a grant, you will be prompted to create a free Submittable account which will allow you to save drafts and view your application status once completed.  This initial screen asks for information required to create your free Submittable account.  Please make sure you are using Chrome, Safari or Firefox browsers.  
 

The form will take approximately 30 minutes to complete and requires you to provide the following information:
 

  1. Date on which the qualified event occurred and proof of the event (newspaper clipping or other independent validation.)
  2. Documents and receipts for expenses incurred as a result of the qualifying event for which you are requesting funding.  Under the terms of the IRS program authorizing disaster relief payments to individuals, all expenses to be reimbursed must the result of the disaster/event, and must be appropriately documented by receipts that include your name, vendor and the amount of the expense. Expenses without receipts will not be reimbursed. (Generally, phone screen shots will not contain enough information to be considered a valid receipt.)

Make sure you answer all required fields and accept the Terms of Use at the end of the form prior to submitting your grant application.  At any time during the process, you can save your application in draft mode by scrolling to the bottom of the form and clicking "Save Draft."  Please note that once you "Submit” your completed application, you will need to contact our support team directly to make any further changes.  
   This application will be treated in a confidential manner by Philanthropi Charitable, and only data required to confirm an your eligibility within your employer's list of approved Charitable Class(es), along with non-identifying information for periodic reporting, will be shared with your employer.
   A confirmation email will be sent to you upon receipt of your application and we expect the review of your application to take about 5-7 days.  If you have questions regarding your application, please send them to grantappsupport@philanthropi.com.
